UCA Forms

Use this form for creating a contract between your congregation and a non-UCA individual or group, for the purpose of regularly renting a hall or church space for a short term eg a weekly dance class. See How To Arrange One-Off Hall Bookings  

Regular non-UCA users will need to obtain their own Public Liability Insurance.

 

Use this form for creating a contract between your congregation and a non-UCA individual or group, for the purpose of renting a hall or church space for one-off events.

Non-UCA one-off users without their own Public Liability Insurance may do so through the Synod NSW/ACT using this form: Hall Hirers Public Liability Application Form. See How to Arrange One-off Bookings.

 

 Use this form:

  1. before a Ministry Agent in a new placement takes up residence to ensure transparency about the condition of the property
  2. annually to keep updated on repairs etc that could be required during the term of the placement.

 

Use this form when a long-term (12 months or more) rental agreement, licence for shared use or lease is being prepared.  Three sets of signatures ae required:

  1. 2 authorised congregation office bearers
  2. the Real Estate Agent or solicitor who prepared the document
  3. Synod Property
